Report a Concern

Please use this form to report a concern for the wellbeing of the client you support, such as not being able to contact them in a while, or if they aren't responding on a pre-arranged date.

Before raising the alarm, please consider any legitimate reasons they may have for not responding. We generally suggest contacting the client at different times over 3 consecutive days to account for their availability. If you know the client is particularly at-risk, or if you have any cause for concern, please don't wait to contact us by submitting this form or calling at 01727 649970. To call out of office hours, use 01727 649985.
This form isn't monitored outside of office hours, so shouldn't replace a 999 call if someone needs immediate medical attention, or a crime is in progress.

Immediate but non-emergency medical advice and guidance from the NHS is available by dialing 111, and non-emergency contact with the police is available by dialing 101.
